(a) An application for a permit to use an existing cabin on state land must (1) be received by the department during a 60-day application period established by the department and announced by public notice; the application period will be open for one time only by geographic location; (2) be made on a form provided by the department; (3) be complete and correct to the best of the applicant's knowledge; (4) contain a description of the cabin and site; (5) contain a description of the proposed use of the cabin and site; (6) be signed and dated by the applicant or, in the case of a group of applicants, by the applicant's authorized agent. (b) An application made under this section must include the following: (1) a $25 application fee; (2) identification of the cabin site on a United States Geological Survey map at 1:63,360 scale (1" to 1 mile) or its equivalent; (3) a photograph (print) of the cabin taken within the past year; (4) documentation demonstrating historic use and occupancy; (5) documentation of any prior investments in the cabin; and (6) any other information required by the director.
