Verification of licensed work experience or training of an applicant required under this chapter must be certified or notarized documentation supplied to the board by  
		(1) another jurisdiction's licensing board;  
		(2) a licensed school or shop;  
		(3) a licensed instructor;  
		(4) a licensed hairdresser, esthetician, barber, nail techician, body piercing practitioner, or tattooing and permanent cosmetic coloring practitioner  who sponsored an apprentice and maintained the required records under 12 AAC 09.190; or  
		(5) an individual who has direct personal knowledge of the applicant's work experience hours.
