Section 12.30.100. License by credentials.

An applicant for a license by credentials shall submit  
		(1) a completed application on a form provided by the department;  
		(2) the applicable application and license fees established in 12 AAC 02.200;  
		(3) verification of a current license to practice as a dispensing optician issued by another licensing jurisdiction that meets the requirements of AS 08.71.145; and  
		(4) repealed 6/22/2003;  
		(5) verification of training equivalent to that required by AS 08.71.110(a)(2).
