Section 12.44.545. Reinstatement of lapsed certified registered nurse anesthetist authorization.

  • 	(a)  The board will reinstate an authorization to practice as a certified registered nurse anesthetist that has lapsed if the applicant submits  
    		(1) a completed application for reinstatement, on a form provided by the department;  
    		(2) evidence of the applicant's current certified registered nurse anesthetist national certification;  
    		(3) if the applicant seeks reinstatement of prescriptive authority, evidence of compliance with the continuing education requirements in 12 AAC 44.525(e);  
    		(4) except as specified  in (c), (d), or (e) of this section, evidence of at least 320 hours of clinical practice as a certified registered nurse anesthetist during the two years before application for reinstatement; and  
    		(5) the biennial certified registered nurse anesthetist authorization renewal fee established by 12 AAC 02.280.  
    	(b)  Hours of clinical practice at the advanced practice level may be counted for reinstatement of both the applicant's registered nurse license and certified registered nurse anesthetist authorization.   
    	(c)  An applicant who does not meet the clinical practice requirement under (a)(4) of this section, and has had no clinical practice as a certified registered nurse anesthetist for at least the two years, but less than five years, immediately preceding the date that the reinstatement application is filed with the department, must complete 200 hours of certified registered nurse anesthetist practice under the direct supervision of a preceptor who is a licensed certified registered nurse anesthetist or licensed physician. At the completion of the supervised practice, the preceptor must submit a statement assessing the applicant's knowledge and ability as a certified registered nurse anesthetist. The lapsed authorization will not be reinstated unless the preceptor states that the applicant has demonstrated competence to practice as a certified registered nurse anesthetist.   
    	(d)  An applicant who does not meet the clinical practice requirement under (a)(4) of this section, and has had no clinical practice as a certified registered nurse anesthetist for at least the five years, but less than 10 years, immediately preceding the date that the reinstatement application is filed with the department, must complete 300 hours of certified registered nurse anesthetist practice under the direct supervision of a preceptor who is a licensed certified registered nurse anesthetist or licensed physician. At the completion of the supervised practice, the preceptor must submit a statement assessing the applicant's knowledge and ability as a certified registered nurse anesthetist. The lapsed authorization will not be reinstated unless the preceptor states that the applicant has demonstrated competence to practice as a certified registered nurse anesthetist.   
    	(e)  An applicant who does not meet the clinical practice requirement under (a)(4) of this section, and has had no clinical practice as a certified registered nurse anesthetist for the 10 years or more immediately preceding the date that the reinstatement application is filed with the department, must complete 500 hours of certified registered nurse anesthetist practice under the direct supervision of a preceptor who is a licensed certified registered nurse anesthetist or licensed physician. At the completion of the supervised practice, the preceptor must submit a statement assessing the applicant's knowledge and ability as a certified registered nurse anesthetist. The lapsed authorization will not be reinstated unless the preceptor states that the applicant has demonstrated competence to practice as a certified registered nurse anesthetist.   
    	(f)  In this section, "clinical practice"  
    		(1) means independent clinical practice as a certified registered nurse anesthetist, providing health care or related activities that have a clinical focus and are at an advanced practice level;   
    		(2) includes teaching, consulting, supervision, and research related to the role of a certified registered nurse anesthetist.
