An application for licensure by credentials as a physical therapist or a physical therapy assistant must be submitted to the department on a form prescribed by the board with (1) all items required under 12 AAC 54.030(a); (2) a report sent directly to the department from the testing organization of the applicant's national physical therapy examination scores; (3) verification of the applicant's licensure status sent directly to the department from each jurisdiction where the applicant holds or has ever held a license to practice physical therapy, one of which must indicate a current license in good standing; each verification from each jurisdiction where the application has or has ever held a license must include an explanation of any disciplinary actions taken against the license; (4) other evidence considered necessary by the board to determine if the requirements of the other jurisdiction were, at the date of issuance, substantially equal to the requirements of this state; (5) verification sent directly to the department from the source that the applicant has (A) been employed in physical therapy at least 60 hours within the 24 months immediately preceding the date the application is received; (B) passed the national physical therapy examination within the 24-month period immediately preceding the date the application is received; or (C) satisfactorily completed an internship of 150 hours approved by the board.
