(a) An applicant for a temporary license to practice as a veterinarian shall submit (1) a completed application for a temporary license on a form provided by the department; (2) the temporary license fee established under 12 AAC 02.350; (3) a notarized copy of the applicant's veterinary school diploma or Education Commission for Foreign Veterinary Graduates certificate; and (4) a statement signed by the supervising veterinarian, on a form provided by the department, indicating that the applicant will be employed under the veterinarian's supervision until the applicant's examination results for the examination under 12 AAC 68.015(b) are reported with a passing score of at least 90 percent. (b) Repealed 5/30/2015.
