A grantee shall have a written evaluation plan that includes (1) an assessment of the attainment of goals and objectives; (2) designation of persons responsible for conducting the evaluation, including the governing body's role; (3) description of the types of data collected; (4) identification of factors which are important in contributing to the success of the program; (5) an assessment of how the grantee affects the community and clients it serves; and (6) a means for the evaluation findings to be used in the planning process described in 13 AAC 90.030.
