(a) A sanitary survey inspector who seeks to renew an approval to conduct sanitary surveys must (1) submit a completed application on a current form supplied by the department; (2) pay the fee required by 18 AAC 80.1910(a)(8); (3) submit verification that the applicant has completed an approved advanced refresher sanitary survey training program for public water systems and passed the written sanitary survey examination with a score of 70 percent or more; (4) provide a list of all sanitary surveys completed by that inspector since the initial approval, or the last renewal, including the date of each survey, the public water system name, and the public water system identification number issued by the department; and (5) provide a written update of the information previously provided under 18 AAC 80.435(b)(5) or this subsection, (b) The department will notify the inspector within 30 days after receipt of a complete application packet required in (a) of this section of the department's approval or rejection of the renewal request. (c) Unless revoked under 18 AAC 80.439, a renewal of an approval issued under this section is valid for two years. (d) A person aggrieved by a decision under this section may request a hearing under 18 AAC 80.1920.
