Section 20.25.320. Filing of forms.

Latest version.

	(a)  If this chapter requires a form to be filed or submitted, one originally signed, commission-furnished form must be submitted to the commission with the required number of copies indicated on each form. A computer-generated form may be submitted in place of the form furnished by the commission if the computer-generated form contains the same required information and has been approved for format by the commission.  
	(b)  Forms required by this chapter will be furnished by the commission upon request and will be revised at the discretion of the commission.
