Prior to investigation, the ombudsman shall ask the complainant for consent to disclose the complainant's identity and relevant information to the agency or agencies that are the subject of the complaint. Consent may be verbal, but must be documented by the ombudsman. The ombudsman has the discretion to request that the complainant provide written, signed consent to the disclosures.
