Section 7.05.330. Authorized copies.

Only one original certificate shall be made for each event, except as otherwise authorized by the State Registrar. Such original shall contain the required signatures and shall be filed, recorded, and forwarded to the bureau for registration. In the case of hospitals, other institutions, and in other specified circumstances the State Registrar may authorize the making of carbon copies at the same time as the original; however, in all cases only the original shall be made on a form marked "Original."
