Section 7.05.695. Report of induced termination of pregnancy.

(a) The form titled Report of Induced Termination of Pregnancy, dated July 2005, is adopted by reference. This form is prepared by the department and contains the information required by AS 18.50.245(e). (b) Except as provided in AS 18.50.310, the report in (a) of this section and information collected by the department from completed reports is confidential. Inspection, disclosure, or copying of completed reports will not be permitted. Completed reports may be used only for statistical purposes. (c) The report shall be completed in accordance with AS 18.50.245 and meet the requirements of (d) of this section. The original of the completed report shall be submitted to the state registrar within 30 days after the induced termination of pregnancy is completed. (d) The information contained on the completed report must be (1) typed or printed in ink; and (2) submitted on the version of the report adopted by reference in (a) of this section. (e) In accordance with AS 18.50.245(d), the state registrar will destroy completed reports submitted under (c) of this section immediately after the annual statistical report is prepared. The state registrar will retain no copies of the completed reports.
