Section 7.18.220. Individual monitoring in controlled area.

(a) A registrant shall maintain records showing the ionizing radiation doses of all individuals for whom personnel monitoring is required under 7 AAC 18.280. The registrant shall keep these records on an annual occupational radiation exposure form issued by and available from the department with the information required on that form, or on clear and legible records containing all the same information required on the form. The dose information entered on the form or records must be for increments of time not exceeding 12 consecutive months. The information must include the (1) name and date of birth of the individual; (2) name of the registrant; (3) dose equivalents for periods of one month or a calendar quarter, including (A) deep dose equivalent; (B) lens dose equivalent; (C) shallow dose equivalent whole body; (D) shallow dose equivalent maximum extremity; (E) committed dose equivalent; (F) committed dose equivalent maximally exposed organ; (G) total effective dose equivalent; and (H) total organ dose equivalent. (b) Annual occupational radiation exposure information must be compiled annually and incorporated into the individual's occupational radiation exposure history form required to be maintained under 7 AAC 18.340. (c) For the purpose of calculating the effective dose equivalent, the values of the weighting factor (WT) are as follows: Organ Dose Weighting Factors Organ or tissue WT Gonads 0.25 Breast 0.15 Red bone marrow 0.12 Lung 0.12 Thyroid 0.03 Bone surfaces 0.03 Remainder1 0.30 Whole Body2 1.00 1 0.30 results from 0.06 for each of five "remainder" organs, excluding the skin and the lens of the eye, that receive the highest doses. 2 For the purpose of weighting the external whole body dose (for adding it to the internal dose), a single weighting factor, WT=1.0, should be used. (d) A registrant shall retain and preserve records used in preparing the annual occupational radiation exposure form indefinitely, or until the registrant terminates operation and transfers personnel monitoring records to the department as required under 7 AAC 18.340.
