A provider of screening services under AS 18.15.205 and 7 AAC 27.630 shall submit the following information to the department semiannually on a screening report form provided by the department: (1) the identifier for the facility where the child was born; (2) the identifier for the facility where the child was screened if that facility is not the facility where the child was born; (3) the total number of children born at the facility; (4) the total number of children that pass the screening by pulse oximetry for critical congenital heart disease; (5) the total number of children with abnormal screening results; (6) the number of children whose parents refused to have their child screened during the newborn period.
