Section 7.34.080. Fees.

Latest version.

	(a)  A patient applying for the first time for a registry identification card under 7 AAC 34.020 must submit a non-refundable application fee of $25.  
	(b)  Except as otherwise provided in 7 AAC 34.070(b), a patient applying for renewal of a registry identification card under 7 AAC 34.070 must submit a non-refundable application renewal fee of $20.
