(a) An agency shall make diligent efforts to involve the following individuals in planning the discharge of a child: (1) the child; (2) the child's parent; (3) child placing workers in the agency; (4) foster parents or residential staff responsible for the child's care; (5) the state placing worker, if applicable. (b) At discharge, the agency shall include the following in a child's record: (1) a discharge summary showing services provided during care, the growth and accomplishments, assessment of needs that remain to be met, and recommendations about the services needed to meet these needs; (2) date of discharge, reason for discharge, and the name and relationship of each person or agency to whom the child was discharged; (3) aftercare recommendations.
