Within 60 days after the end of each calendar quarter, the administrator shall submit to the department, on a form supplied by the department, the following information regarding patients admitted to the facility during the previous quarter who were found to be suffering from a mental illness and who posed a danger to self or others, or who were gravely disabled, as determined by each patient's treating physician or psychiatrist: (1) the number of patients admitted voluntarily; (2) the number of patients admitted involuntarily; (3) the number of patients admitted who had insurance or self-pay coverage that was billed; (4) the average length of stay for all patients admitted during the quarter, whether voluntary or involuntary; (5) the number of patients who were readmitted during the quarter.
