(a) The division will release client information to a person other than the client or the client's parent, guardian, or other representative, if (1) release is for a purpose directly connected with the administration of the vocational rehabilitation program only; and (2) the client or, as appropriate, the client's parent, guardian, or other representative consents in writing. (b) Personal information released under this section may be used only for the purpose for which it is provided and may not be further released to any person or agency. The person to whom it is released must provide written assurance to this effect. (c) If information requested has been obtained from another agency or professional, it will be released only under conditions established by that agency or professional. (d) The division will charge $.25 per page for copies of any document released under this section.
