(a) For a permit with a term of more than one year, the department may require (1) the permittee to file information the department determines necessary to show compliance with the permit at the end of each year's authorized operating period; and (2) operational information from the permittee, including the number of clients served, number of visitor days, and number of days in operation. (b) In a permit, the department may require the permittee, upon completion of the operations under the permit, to file a completion statement, including a detailed description of cleanup and restoration work at the site. The department may also require the permittee to provide a map showing the location of all permit activities. The permittee shall complete any additional cleanup and restoration work that may be required.
