Section 12.44.785. Reinstatement of a revoked license or authorization.

(a) One year after revocation of a license or authorization, the nurse, certified registered nurse anesthetist, or advanced nurse practitioner may apply to the board in writing for reinstatement. (b) The applicant for reinstatement shall appear before the board. (c) The board may impose restrictions upon the nurse, certified registered nurse anesthetist, or advanced nurse practitioner when reinstating a license or authorization. (d) The applicant is responsible for providing a written evaluation from the employer regarding his or her performance and will be evaluated by the board to determine whether or not the license or authorization will be reinstated.
