(a) An operator who seeks renewal of a lapsed certificate must submit a request for renewal to the department within 180 days after the certificate lapses. The department will renew a lapsed certificate upon receipt of a request for renewal that includes proof of compliance with 18 AAC 74.810. An individual with a lapsed certificate is not certified. (b) The department will require reexamination of an operator whose renewal application is received more than 180 days after the certificate lapses, if the department determines that reexamination is necessary to protect human health, safety, and welfare. The department will consider board recommendations in determining the need for reexamination under this subsection. (c) The department will not renew a certificate that has lapsed by more than two years. An individual with a certificate that has lapsed by more than two years must seek recertification under 18 AAC 74.020 or 18 AAC 74.420.
