(a) A certified emergency medical service providing either basic life-support or advanced life-support outside a hospital must complete an approved EMS report form for each patient treated. The report form must document vital signs and medical treatment given the patient. (b) A copy of the completed EMS form must (1) accompany the patient to the treatment facility; (2) be sent to the medical director; and (3) be kept by the EMS service as a permanent record. (c) The medical director shall periodically review the EMS reports he or she receives, to determine the appropriateness of treatment given.
