(a) An instrument, contract, or document form which an employment agency requires an applicant or his legal representative to execute or become a party to may not be used unless its form and content has been approved by the director. (b) An employment agency shall submit two copies of each form described in (a) of this section to the department for approval which will be indicated by an endorsement by the director. One copy will be returned to the employment agency to be kept as part of its record. Approval will not be withheld unless the instrument, contract, or document is vague, makes misrepresentations, is unfair, oppressive, or contains provisions which violate an employment agency law or regulation.
