Section 7.18.380. Report of overexposures and excessive levels and concentrations.

(a) In addition to the notification required under 7 AAC 18.370, a registrant shall submit a detailed report in writing to the department not later than 30 days after an incident (1) for which notification is required under 7 AAC 18.370; (2) in which an exposure occurred of an individual to ionizing radiation in excess of an applicable limit as set out in 7 AAC 18.200 - 7 AAC 18.390 or as otherwise approved by the department; and (3) in which levels of ionizing radiation not involving excessive ionizing radiation doses occurred to any individual in an uncontrolled area in excess of 10 times any applicable limits as set out in 7 AAC 18.200 - 7 AAC 18.390 or as otherwise approved by the department. (b) A report required under this section must describe the (1) extent of exposure of each individual to ionizing radiation; (2) levels of ionizing radiation; (3) cause of the exposure or levels; and (4) corrective steps that were taken or planned to prevent a recurrence. (c) If a registrant is required under this section to report to the department any exposure of an individual to ionizing radiation, the registrant shall, not later than the delivery of the report to the department, also notify the individual of the nature and extent of exposure. The notice must be in writing and contain the following statement: "This report is furnished to you under the regulations of the Department of Health and Social Services. You should preserve this report for future reference." (d) A report filed with the department under this section must be prepared in a manner that the name of an individual who has received exposure to ionizing radiation is stated in a separate part of the report.
